To continue providing high-quality care while managing rising operational costs, Dr. Amy D. Miller & Associates, Ltd. is updating its payment policy. Effective July 1, 2026 a 3% surcharge will be applied to all payments made by credit card.
This surcharge is established to cover most of the processing fees (currently 3.3% + .30 transaction fee) charged to us by Square (payment processor).
How to Avoid the Surcharge:
You can avoid this fee by paying your balance using any of the following methods:
- Zelle (Use: 847-845-4045 Amy Miller)
- Cash
- Personal Check
- Health Savings Account (HSA) or Flexible Spending Account (FSA) card
Please note, although we are still charged processing fees for HSA cards and FSA cards these payment types are not subject to the 3% surcharge due to regulations. Please pay by other methods when possible.
